Please talk to one of our Customer Service Representatives or Sales Personnel before you take the time to send us a completed credit application. Customer Service can be contacted by filling out the form on the information request page or by calling 1-800-323-8562.

The Credit Department joins with Sales and Customer Service in welcoming you as a new Viskase Customer.

In order to establish a permanent credit arrangement to meet your requirements, we require additional information regarding your company.

Please complete and return the Credit Application (all information is necessary) and the Sales/Use Tax Exemption Certificate to the fax number listed on the form. Please provide three trade references that supply you with products other than "perishable" food items. All information you provide will be held in the strictest of confidence.
